Note: Cheques / Pay Order / Bank Demand Draft should be in favour of "G.D. Goenka Public School, Ludhiana" Payable at Ludhiana. Fee revision may be to an extent of 8-10% annually as may be determined by the school board of management.

INSTRUCTION FOR PARENTS
	This is an application for registration only. Those selected for interview will be informed by post.
	Original Municipal Birth Certificate is to be submitted at the time of Admission (For nursery & K.G.)
	Please fill-up form very carefully, as selection for interaction will be based on the information given herein.
	Incomplete form without required documents & signature will be cancelled.
	It will be the responsibility of the parents to see that the registration form is complete in all respect. The Registration Form if incomplete in any aspect will be rejected.
	The parents are responsible for Correctness of date entered in the Registration Form. Registration and even admission will be cancelled if any information recorded therein is found to be false.
	The Admit Card/Interview Card, with the Registration Form must be brought at the time of observation of the child without which the Child will not be allowed for the same.
	The Registration Form is to be filled and submitted to the Administrative Office within five days of issue of the Registration Form.
	Incomplete or illegible Registration Form, without photograph will not be accepted.
	Date for Tests / Observation and interaction will be given at the time of registration and the same may be changed without prior notice, by the school.
	Registration does not imply admission. Admission is subject to the results of the Admission Test, observation interaction and the availability of seats.
	At the time of Registration, the following documents along with the Registration Form are to be submitted at the Administrative office. a. Completed and signed Registration Form b. Three passport-size photographs (colored) pasted at the appropriate place in the Form. c. Photocopy of proof of Date of Birth (issued by the Municipal Corporation or any Competent Authority) of candidates seeking admission to classes Nursery to Class I. Date of Birth Certificate issued by Doctor/Nursing Homes/ Hospital will not be accepted.
	Admission to Nursery and K.G. Classes will be through observations/interaction only.
	There will be a Written Test for students seeking admission to Classes I-VII.
	Both parents should be present at the time of observations/interaction.
	The parents must fill the registration form themselves. In case the form is filled by guardian, it should be clearly indicated.
	Care must be taken to ensure that the Date of birth filled in the Registration Form tallies with the date on the original Birth Certificate.
INSTRUCTION FOR CANDIDATES
	Both copies of Admit card to be brought at the time of the Admission test.
	Candidates should reach the venue 30 minutes before the schedule time to allow for the time to settle in.
	Candidates should bring their own stationery like pencils/pens/color pencils/geometry box etc.
